A needs assessment is a meeting between you and a care professional to understand what type of support you may need to live independently.

 

About the social care needs assessment

We understand you want to continue living in your own home, staying as independent as possible, but sometimes you might need some extra support.

That’s why we can support you to:

  • learn new skills and abilities
  • stay connected with your local community, family and friends
  • enjoy hobbies and interests
  • make the best use of equipment and technology.

If you feel you need some extra support, or would like to discuss options available, you can ask us for a needs assessment.
